Cherokee Home Health
The mission of Cherokee Home Health is to assist people of all ages on the Cherokee Indian Reservation to achieve their health care goals by providing compassionate, individualized, quality care, with respect for cultural beliefs.

Scope of Work: The Home Health Program provides health services to clients that are permanently or temporarily homebound, or where the home is the most appropriate setting if allowed by payer source. Services include: Skilled Nursing, Certified Nursing Assistants, In-Home Aides, Physical Therapy, Occupational Therapy, and Speech Therapy, and supplying adult diapers and blue pads to eligible Medicaid recipients.

2004 Accomplishments:

  • Increased the average monthly patient census by 16%
  • Home Health RNs are currently providing case management of the Caring for the Caregiver Program, in collaboration with Tsali Manor. Respite care is offered as part of this, provided by Home Health CNAs, In-Home Aides, and contract staff, including companions.
  • The Home Health office was relocated to the building on Children’s Home Loop, previously used by the Behavioral Health Program.
  • Participated with Community Health, other Tribal programs, and the CIHA to provide flu vaccine to current patients, employees and at risk members of the community.
  • Reduced vacant nursing assistant positions by 50%.
  • Received renewal certificate of accreditation through Accreditation Commission of Health Care.
  • Field Staff RN’s began using laptop computers for patient admissions and re-certifications.
  • Over 3000 home visits provided from January thru August 2004.
  • Over 10,000 units (15 minute increments) of Personal Care Services provided.
  • Over 13,600 units of CAP (Community Alternative Plan) Services provided.
  • Quality Improvement Supervisor has been hired; QI plan revised and implemented.
  • Implemented Outcomes Based Quality Improvement Plan, with participation of all staff to educate patients and caregivers on ways to reduce unnecessary visits to the Emergency Room.
  • Created new system to track the management of slow healing wounds.
  • Improved system to have physician orders signed and returned within 30 days.
