The mission of the Community Health Nursing is to provide quality nursing services in the community through skilled nursing assessments and interventions, patient education, and collaboration with other programs.

Scope of Work: Services provided by Community Health Nursing include: home visits to elderly and vulnerable population, in-home follow up of newborns with health problems,
assistance with medications, pre-drawing syringes, STD/HIV-AIDS counseling, tracking of communicable diseases, patient and parenting education, Eye Clinic, etc.

2004 Accomplishments:

  • We have implemented a formal charting system that integrates CHN/CHR services.
  • We have increased our participation in health fairs and community activities providing blood pressure and blood sugar checks, as well as community health education.
  • Staff has received training for RPMS for PCC data entry, enabling physicians at Cherokee Indian Hospital to access patient information concerning services provided by Community Health Nursing and the CHR program.

2004 Accomplishments: continued

  • Have expanded the number of worksite screenings offered to Tribal Programs.
  • Interfaced with Cherokee Home Health to provide continuous service to patients served by both programs, depending on patient needs.
  • Established first Community wide Health Outreach Screen, in the Wolfetown Community.
  • Relocated to more central location, on Children’s Home Loop, in the former Behavioral Health building.
  • Community Health participated with other tribal programs, county, and state agencies in the disaster drill held at the Bingo Hall. Assessments and triage were performed by Community Health and Home Health nursing staff.
  • Community Health Nursing staff set up disaster training exercise, conducted by the State Surveillance Team.
  • Community Health Manager and Supervisor have completed four week course in Public Health Nursing provided by UNC School of Public Health.
  • Nursing staff is participating with Public Health Nurse at CIHA to establish Epidemiological Team.
  • January through August 2004 community health nursing has made 1151 home visits.
