Community Health Representatives
  The mission of Community Health Representatives is improve the health of members of the Eastern Band of Cherokee Indians by providing quality outreach health care services and health promotion/disease prevention activities for the community.

Scope of Work: The Community Health Representative program provides health
services to members of the Eastern Band of Cherokee Indians in a culturally accommodating manner. The program provides outreach, early detection and prevention through culturally congruent care patterns.

2004 Accomplishments:

  • CHR/C.N.A. staff are averaging over 400 home visits per month.

  • All CHR positions are fully staffed.
  • Staff has received training for RPMS for PCC data entry, enabling physicians at Cherokee Indian Hospital to access patient information concerning services provided by Community Health Nursing and the CHR program.
  • CHR continue to increase their participation in health screens.
  • Have crossed trained two additional CHR’s for Eye Clinic administrative duties.
  • Eye Clinic staff have successfully switched over to the new scheduling menu on RPMS.
  • Two CHRs have successfully completed the training on the new RPMS electronic referral system.
  • All CHR and C.N.A. staff attended either the WNC C.N.A. conference at Lake Junaluska, or the C.N.A. Hands On Skills workshop in Hickory.
